This online self-declaration form can only be filled out and submitted by adult audience members. Declarations for minors must be submitted in person by their legal guardian. This online self-declaration cannot be submitted earlier than 48 hours before the curtain-up. Please consider submitting your health declaration online. This way you will save time and will not have to fill out the declaration when entering the opera house.
